Here you'll find all the required child care paperwork in one place. 

The attached forms in the Parent Packet are required by the Wisconsin Department of Health and Family Services for your child's enrollment.  They should be completed and returned no later than one week before the child's first day of attendance. The packet includes a child health history and emergency plan, youth membership application, a parent release online payment instructions, our enrollment agreement and other documents. Please complete and return to your child care center office. Everyone enrolling, no matter the age, needs to have this paperwork completed.

Is your child under age 2? Please also complete the Intake Form here    
This form collects information about children under age 2 in order to aid child care workers in individualizing the program of care for the child in a family or group child care center.

Is your child entering school age care?  Please complete the transportation permission form, alternate arrival release agreement, School Days Out & Snow Days Enrollment Agreement and pre-registration form in our School Age Packet here.  Please complete and return to your child care center office.

Please read our Parent Handbook that describes our policies on attendance, tuition, nutrition, medicine and much more, along with parent/guardian rights and responsibilities to ensure your child's success. Fees are also listed at the end of the handbook or with the specific child care program type.
